needs intermittent skilled visits. All services are performed under the
direction of the physician, which can be coordinated through the hospital
discharge planning department, the insurance company or the physician's
office.

continuity of care by working closely with the client's physician.
Services are coordinated to meet the individual needs of the client and
the physician's plan of care.
Availability: A registered nurse is on-call 24 hours a day, 7
days a week.
